Registration Information for the 2010 season.
Registration is pretty straight forward. As with any club sport, there are some
forms to fill out, get signed, and turned in. However, unlike other sports, you
are required to be a member of US Lacrosse. This can be accomplished by going to
their site and going through the registratio process for that organization. You

page.
Below are the documents that you need in order to play lacrosse with Cougar Lacrosse.
Each form can be printed and turned in to a coach or to a board member at a practice,
meeting, or registration event.

Why you need it

Registration Form
|
This is the registration form with all of your pertinent information. It is required
for all players.

Medical Form
|
This form is required to ensure each player is fit to play lacrosse.
|
|
Physicians Statement
|
This form is only required if the parent wishes the player to undergo a physical
by a licensed professional. The medical form contains language stating that you
certify that your child is fit to play lacrosse.

|
Code of Conduct
|
Required form stating that the player and parent has read and understands the code
of conduct expected from them during any Cougar Lacrosse Event.
